A Meaningful Goal Housing Shelter helps the homeless to rehabilitate lives and become part of the working society. .Intake. Hr. 6 am to 10 pm. .
Cost is $650.00 A month and Deposit is Required.
Transitional Independent Living Program

Clients accepted into the transitional program are provided support, counseling, job training and referrals.  Each client works with a team of professionals to work through a personalized strength-based program that will prepare them for employment and independent living.

The program provides:

    Strong focus on education and career achievement
    Referrals to permanent housing
    Furnished living quarters
    Life-Skills training, support, and advocacy
    Encouragement, recognition, respect, accountability, and a safety net
    Bi-monthly educational and fun activities

The Process of Change is a 3-year program which guides individuals through the initial stages of attaining housing and employment, then provides ongoing management and follow-up to ensure sustained success.

Process of Change Program Phases

The Process of Change (POC) is a 3-year program which guides and mentors clients as they secure employment and permanent housing.    The steps of the process are described below.
Outreach

Clients accepted into POC generally come in through one of our existing service channels:  DCFS, our Beauty Program, Transition Resource Center, IP Living homes and area homeless shelters.
Orientation

Interested clients receive and overview of program expectations and then are interviewed to determine their eligibility for the POC pogram.   Individuals are evaluated based on necessity, personal motivation, and their suitability to the program.   Clients accepted into the program then enter into a 3-year, 4-phase process.
Level I -Preparation (Months 1-6)

    Assess needs and goals
    Individual/group therapy
    Scattered site housing
    Case management
    Employment training/ internships
    Education
    Independent living skills training
    Transportation
    Clothing
     youth advocacy

Level II Implementation (Months 7-12)

    Ongoing mental health therapy
    Ongoing management
    Ongoing assessment as needed
    Maintained resources
    Continued independent living training
    Continued support services
    Maintained employment/ internships

Level III Independent Transition (Months 13-24)

    Assistance with off-site housing applications
    Ongoing mental health therapy
    Ongoing case management
    Ongoing assessment as needed
    Maintained resources
    Continued independent living training
    Continued support services
    Manintained employment/internships
    Continued supportive services as needed
    Assistance with moving/transitioning

Aftercare Follow-up (Months 25-36)

    Permanent housing
    30 day follow- up
    60 day follow-up
    90 day follow-up
    120 day follow-up
    9 month follow-up
    1 year follow-up
    Provide supportive services as needed
